Tirzepatide, explained

A drug studied for blood sugar and body weight.

Published phase 3 trialsRead the sources

What it’s used for

Human study

Weight loss

At 72 weeks, average loss was 15.0%, 19.5% and 20.9% with 5, 10 and 15 mg, versus 3.1% with placebo.

Approved medicine context

Blood-sugar control

Mounjaro is approved to improve blood sugar in type 2 diabetes.

Approved medicine context

Sleep apnea

Zepbound is approved for moderate-to-severe obstructive sleep apnea in adults with obesity, alongside diet and exercise.

Human study

Heart outcomes

A large trial found heart events in 12.2% versus 13.1% with dulaglutide: not worse, but not proven better.

Community reports

Appetite

People report less appetite and feeling full sooner.

What to expect

  1. Days 1–3

    Appetite drops

  2. Weeks 1–2

    Blood-sugar changes

  3. Weeks 2–4

Stomach and gut adjust; early nausea is common

  4. Months

Weight loss builds over months, not at a set weekly rate

  5. Week 72

15.0–20.9% average loss across doses in a large trial

Side effects & safety

Side effects

  • Nausea, diarrhea, vomiting, constipation and abdominal discomfort
  • Label warnings include pancreatitis, gallbladder disease and dehydration-related kidney injury
  • Severe gut reactions, retinopathy (eye damage) and aspiration are also listed risks

Stop and get help if

  • Severe abdominal pain
  • New symptoms in the neck
  • Vomiting that won't stop
  • Passing very little urine
  • Swelling (edema)
  • Confusion
  • Signs of an allergic reaction
  • Severe mood changes

Avoid or check first

  • Personal or family history of medullary thyroid cancer (MTC) or MEN2 — contraindicated
  • Taking insulin or sulfonylureas — higher risk of low blood sugar
  • Using oral birth control — the label advises a non-oral or added barrier method for 4 weeks after starting and after each dose increase

In sport

  • The reference describes tirzepatide as not prohibited in 2026 or 2027, but monitored by WADA.

How to mix5 steps
  1. Wipe both tops

Wash your hands, then swab both rubber vial tops with alcohol. Let them dry.

  2. Draw the water

Use a new sterile syringe to draw the water volume selected in the calculator.

  3. Run it down the wall

Introduce the water slowly down the inside glass wall, rather than directly onto the powder.

  4. Swirl, don’t shake

Gently swirl or roll the vial. Allow time for the powder to dissolve.

  5. Label the vial

Write the preparation date and the amount and final volume on the vial label.

Injecting under the skinWhere, rotating sites, which syringe

Where

Subcutaneous injections go into the fatty layer beneath the skin. Common sites include the abdomen, outer thigh and upper arm. Keep at least 5 cm from the navel and avoid bruised, irritated or damaged skin.

Rotate sites

Change the spot each time. Clean the skin and let it dry.

Single-use equipment

Use a new sterile needle and syringe each time. Place used needles in a sharps container.

Storage

Storage

Refrigerated at 2–8°C.

Combining it with others

Don’t combine

  • Semaglutide

Similar appetite-hormone effects may add to stomach problems and dehydration.

  • Retatrutide

Similar appetite-hormone effects may add to stomach problems and dehydration.

  • Survodutide

Similar appetite-hormone effects may add to stomach problems and dehydration.

  • Mazdutide

Similar appetite-hormone effects may add to stomach problems and dehydration.

  • Orforglipron

Similar appetite-hormone effects may add to stomach problems and dehydration.

Use caution

  • Birth control pills

Birth control pills may work less reliably when starting or increasing the dose.

  • IGF-1 LR3

Both lower blood sugar, which may increase the risk of it falling too low.

  • MK-677

Increased hunger and blood sugar can work against the appetite-hormone effects.

  • GHRP-6

One increases hunger while the other reduces it.

  • Tesofensine

Concerns about heart rate, blood pressure and eating too little may add up.

  • Adipotide

Kidney injury reported in animals may add to kidney strain linked to dehydration.

  • Melanotan I

    Nausea may get worse.

  • Melanotan II

    Nausea may get worse.

  • PT-141

    Nausea may get worse.

  • Cagrilintide

Both slow stomach emptying and reduce appetite, which may add to nausea.

  • Eloralintide

    Slower stomach emptying.

What the research says

Two hormone targets

Tirzepatide acts on the appetite-hormone signals GIP and GLP-1. Other drugs target just GLP-1 or three hormone signals.

What studies found

SURMOUNT-1

Human study · 2,539 adults · 72 weeks

In 2,539 adults without diabetes, average weight changes at 72 weeks were −15.0%, −19.5% and −20.9% across the three tirzepatide groups, versus −3.1% with placebo. Reported health problems and treatment stops were also recorded.

Longer prediabetes follow-up

Human study · 1,032 adults · 176 weeks plus 17 weeks off treatment

In 1,032 adults with obesity and prediabetes, average weight loss at 176 weeks was 12.3–19.7%, versus 1.3% with placebo. Diabetes developed in 1.3% versus 13.3%. After another 17 weeks off treatment, those figures were 2.4% versus 13.7%.

How long it stays in your body

Half-life is the time it takes for the amount in the body to fall by half. It does not tell you how long effects last.

Questions

What does 10mg mean?

It describes an amount of material in the unit stated on the label. It does not describe liquid volume, a dose, or how to prepare the product.

Read more about amounts
Does purity mean it is sterile?

No. Purity and sterility require different tests. An endotoxin result also does not establish sterility.

About testing
Can I calculate concentration?

You need a mass in mg and either a final volume or a target concentration. IU and blends require separate, verified information.
