Retatrutide, explained

An experimental weekly injection being studied for weight loss, appetite and blood sugar.

Published phase 2 and phase 3 trialsRead the sources

What it’s used for

Human study

Weight loss

Average weight loss was 25.0% over 80 weeks with 12 mg, versus 3.9% with placebo.

Human study

Blood-sugar control

Long-term blood sugar (HbA1c) fell 1.94 percentage points at 40 weeks with 12 mg, versus 0.81 with placebo.

Community reports

Less appetite

People report less hunger and easier appetite control.

Research topics

Other metabolic markers

Research also covers blood pressure, blood fats, liver fat and insulin sensitivity. Findings differ by outcome.

What to expect

  1. Weeks 1–4

    Appetite drops

  2. Following months

    Weight keeps changing

  3. Week 48

24.2% average loss on 12 mg in the phase 2 trial

  4. Week 80

25.0% average loss on 12 mg in the phase 3 trial

Side effects & safety

Side effects

  • Stomach and gut symptoms — the most common side effects, and more frequent at higher doses
  • Faster heart rate, rising with dose
  • Altered skin sensations

Stop and get help if

  • Severe abdominal pain, especially in the upper right side
  • Repeated vomiting or diarrhea
  • Signs of dehydration
  • Low blood sugar
  • Losing too much weight

What regs say

  • Not FDA-approved, and the FDA warns against unapproved retatrutide products.

Avoid or check first

  • Medullary thyroid cancer (MTC) or MEN2 risk — needs medical assessment first
  • Pregnant — needs medical assessment first
  • Already using semaglutide or tirzepatide — their effects overlap
  • Taking insulin — higher risk of low blood sugar

In sport

  • The reference considers it likely prohibited under S0 while unapproved. Sport Integrity Australia notes it is not named on the list; later approval could change the position.

How to mix5 steps
  1. Wipe both tops

Wash your hands, then swab both rubber vial tops with alcohol. Let them dry.

  2. Draw the water

Use a new sterile syringe to draw the water volume selected in the calculator.

  3. Run it down the wall

Introduce the water slowly down the inside glass wall, rather than directly onto the powder.

  4. Swirl, don’t shake

Gently swirl or roll the vial. Allow time for the powder to dissolve.

  5. Label the vial

Write the preparation date and the amount and final volume on the vial label.

Injecting under the skinWhere, rotating sites, which syringe

Where

Subcutaneous injections go into the fatty layer beneath the skin. Common sites include the abdomen, outer thigh and upper arm. Keep at least 5 cm from the navel and avoid bruised, irritated or damaged skin.

Rotate sites

Change the spot each time. Clean the skin and let it dry.

Single-use equipment

Use a new sterile needle and syringe each time. Place used needles in a sharps container.

Storage

Fridge · 28 days

Refrigerated at 2–8°C. The reported 28-day period has not been tested for every vial.

Dry powder

Room temperature.

Combining it with others

Don’t combine

  • Survodutide

Similar appetite-hormone effects can add to nausea and dehydration.

  • Mazdutide

Similar appetite-hormone effects can add to nausea and dehydration.

  • Orforglipron

Similar appetite-hormone effects can add to nausea and dehydration.

Use caution

  • Insulin or sulfonylureas

Both lower blood sugar, which may make it fall too low.

  • Oral medicines

Slower stomach emptying can change how pills are absorbed.

  • IGF-1 LR3

Both lower blood sugar, increasing concern about it falling too low.

  • MK-677

Increased hunger and blood sugar work against the appetite-hormone effects.

  • GHRP-6

One increases hunger while the other reduces it.

  • Tesofensine

Concerns about the heart, blood pressure and eating too little may add up.

  • Adipotide

Kidney injury seen in animals may add to kidney concerns linked to dehydration.

  • Melanotan I

    Nausea may get worse.

  • Melanotan II

    Nausea may get worse.

  • PT-141

    Nausea may get worse.

  • Cagrilintide

Slower stomach emptying and reduced appetite can add to nausea.

  • Eloralintide

Slower stomach emptying and reduced appetite can add to nausea.

What the research says

Three hormone targets

Retatrutide activates GLP-1, GIP and glucagon receptors.

Human study

Blood-sugar study

TRANSCEND-T2D-1 followed 537 adults with type 2 diabetes for 40 weeks. Long-term blood sugar (HbA1c) fell 1.69–1.94 percentage points across retatrutide groups, versus 0.81 with placebo.

What studies found

Phase 2 weight study

Human study · 338 adults · 48 weeks

The trial followed 338 adults for 48 weeks. At week 24, average weight loss ranged from 7.2% to 17.5%, versus 1.6% with placebo. At week 48, weight changes ranged from −8.7% to −24.2%, versus −2.1%. Stomach symptoms and heart-rate changes were also recorded.

TRIUMPH-1

Human study · 2,339 adults · 80 weeks

In 2,339 adults with obesity and no diabetes, average weight changes at 80 weeks were −17.6%, −23.7% and −25.0% across the three groups, versus −3.9% with placebo. Stomach symptoms were the most common side effects.

How long it stays in your body

Half-life is the time it takes for the amount in the body to fall by half. It does not tell you how long effects last.

Backbone runs from N to C terminus. Aib is alpha-aminoisobutyric acid; alphaMeLeu is alpha-methyl-L-leucine. Lys17 carries a fatty-diacid moiety through a linker; the C terminus is amidated. The token sequence alone does not encode the complete acylated structure.

Molecular notes

Retatrutide / LY3437943. WHO supplies the modified reference molecule’s formula. Using CIAAW abridged atomic weights, 221×12.011 + 342×1.0080 + 46×14.007 + 68×15.999 = 4731.421, rounded to approximately 4731 Da. This average formula mass excludes counterions and is not a measured monoisotopic mass or a Monroe vial assay.

Questions

What does 10mg mean?

It describes an amount of material in the unit stated on the label. It does not describe liquid volume, a dose, or how to prepare the product.

Read more about amounts
Does purity mean it is sterile?

No. Purity and sterility require different tests. An endotoxin result also does not establish sterility.

About testing
Can I calculate concentration?

You need a mass in mg and either a final volume or a target concentration. IU and blends require separate, verified information.
