Semaglutide, explained

A drug studied for blood sugar, weight management and heart-related outcomes.

Published randomized clinical trialsRead the sources

What it’s used for

Human studies

Weight loss

A 2.4 mg weekly trial reported 14.9% loss at 68 weeks versus 2.4% with placebo. A 7.2 mg study reported 18.7% at 72 weeks.

Approved medicine context

Blood-sugar control

Specific approved semaglutide medicines improve blood-sugar control in diabetes.

Approved medicine context

Heart risk

Wegovy is approved to reduce certain serious heart and blood-vessel risks.

Approved medicine context

Fatty liver

Wegovy injection has accelerated approval for adults with an inflammatory fatty-liver condition, MASH, and F2–F3 liver scarring, without cirrhosis.

Community reports

Appetite

People report reduced appetite and cravings, with easier weight maintenance.

What to expect

  1. Weeks 1–4

    Blood-sugar changes

  2. Months 2–6+

    Weight loss builds up gradually

  3. Week 68

14.9% average loss on 2.4 mg in a large trial

Side effects & safety

Side effects

  • Nausea, vomiting, diarrhea and constipation
  • Fatigue, headache and hair loss
  • Altered skin sensations, more common at the 7.2 mg dose along with stomach effects
  • Slower stomach emptying, which can affect how oral medicines are absorbed
  • Label warnings include pancreatitis, gallbladder disease and dehydration-related kidney injury
  • Retinopathy (eye damage) and aspiration are also listed risks

Stop and get help if

  • Abdominal pain
  • Yellowing of the skin or eyes (jaundice)
  • Swelling in the neck
  • Trouble breathing or signs of an allergic reaction
  • Passing very little urine
  • Vision changes
  • Confusion or fainting

Avoid or check first

  • Personal or family history of medullary thyroid cancer (MTC) or MEN2 — contraindicated under a boxed warning
  • Pregnant or planning pregnancy — the label says to stop at least 2 months before a planned pregnancy
  • Taking insulin or sulfonylureas — higher risk of low blood sugar
  • Using another semaglutide or GLP-1 product — taking them together isn't recommended

In sport

  • The reference describes semaglutide as not prohibited, while noting WADA monitoring of GLP-1 medicines.

How to mix5 steps
  1. Wipe both tops

Wash your hands, then swab both rubber vial tops with alcohol. Let them dry.

  2. Draw the water

Use a new sterile syringe to draw the water volume selected in the calculator.

  3. Run it down the wall

Introduce the water slowly down the inside glass wall, rather than directly onto the powder.

  4. Swirl, don’t shake

Gently swirl or roll the vial. Allow time for the powder to dissolve.

  5. Label the vial

Write the preparation date and the amount and final volume on the vial label.

Injecting under the skinWhere, rotating sites, which syringe

Where

Subcutaneous injections go into the fatty layer beneath the skin. Common sites include the abdomen, outer thigh and upper arm. Keep at least 5 cm from the navel and avoid bruised, irritated or damaged skin.

Rotate sites

Change the spot each time. Clean the skin and let it dry.

Single-use equipment

Use a new sterile needle and syringe each time. Place used needles in a sharps container.

Storage

Storage

Refrigerated at 2–8°C.

Combining it with others

Don’t combine

  • Tirzepatide

They affect the same appetite hormone, GLP-1.

  • Retatrutide

They affect the same appetite hormone, GLP-1.

  • Survodutide

They affect the same appetite hormone, GLP-1.

  • Mazdutide

They affect the same appetite hormone, GLP-1.

  • Orforglipron

They affect the same appetite hormone, GLP-1.

Often taken together

  • Cagrilintide

The studied combination uses a different prepared product.

What the research says

GLP-1 signaling

Semaglutide acts on the appetite hormone GLP-1. Tirzepatide acts on both GIP and GLP-1 hormone signals.

What studies found

STEP 1

Human study · 1,961 adults · 68 weeks

In 1,961 adults without diabetes, average weight change at 68 weeks was −14.9% with semaglutide versus −2.4% with placebo. Both groups received lifestyle support. The main analysis included people who stopped treatment or needed additional treatment.

SELECT

Human study · heart disease, no diabetes

SELECT followed 17,604 adults with heart or blood-vessel disease and no diabetes for an average 39.8 months. Death from those diseases, a nonfatal heart attack or a nonfatal stroke occurred in 6.5% versus 8.0% with placebo (risk estimate 0.80). Reported health problems caused permanent treatment stops in 16.6% versus 8.2%.

How long it stays in your body

Half-life is the time it takes for the amount in the body to fall by half. It does not tell you how long effects last.

N-to-C backbone. Aib is alpha-aminoisobutyric acid. The label’s native GLP-1 numbering calls the acylated residue Lys26; it is token 20 of this shorter backbone. The C18 fatty-diacid spacer is separate from the tokens.

Questions

What does 5mg mean?

It describes an amount of material in the unit stated on the label. It does not describe liquid volume, a dose, or how to prepare the product.

Read more about amounts
Does purity mean it is sterile?

No. Purity and sterility require different tests. An endotoxin result also does not establish sterility.

About testing
Can I calculate concentration?

You need a mass in mg and either a final volume or a target concentration. IU and blends require separate, verified information.
